Don't worry, your annual pass is in Disney's computer. You just need to bring photo ID to Guest Services, and they will issue you a new one. I don't believe you can do anything from home.

We always take a picture of the back of our AP with our phone. That way, if we ever misplace it, we can easily have it replaced at any Guest Services desk. We've done this a time or two when one pass was inadvertently left in the room. Rather than go back to get it, we just went to Guest Services and they quickly replaced it. While they can look it up with just your name and an ID, having a picture of the back speeds the process up a bit.
