Is this still a package option? Or do you book your cruise and time at WDW separately?

It's not really an option anymore. However you can sort of make it a "package" by using the DCL transfers, which will take you to or from any of the WDW resorts to the Port or vice versa. For example, you can book a 3 night cruise followed by 4 nights at a Disney resort. You can take the DCL bus to the port from the airport, then after the cruise take the DCL bus to your WDW resort, check in, then when you depart, take DME back to the airport.

The only thing they did away with was the "check in only once/one key for cruise and resort stay" aspect. You can technically book your package through DCL, but it is often a better deal to do it yourself so you can take advantage of any discounts available to you.

But the transfer is $35 per person each way, right? My family of four is taking a towncar from CBR to Port C and back for a grand total of $135 with a private towncar driver.
