How do I address boxes sending ahead of us???

It has been 4 years since our last trip and I can not remember how I addressed our boxes.
I am sending them UPS.

This is what I have so far.

WALT DISNEY WORLD ART OF ANIMATION RESORT
ATTN: JESSICA FLORCZAK
1850 CENTURY DRIVE
LAKE BUENA VISTA, FL 32830

I know I need to get my confirmation number and arrival date on there but can not remember where to put it.

Just be sure that your reservation number and arrival date are clearly marked and easily visible - no special place to put it as long as it is there and won't be missed.
