When you use the online check in can you request a specific building and floor. DH has some medical issues,is there somewhere that you can put that on the online check in form?

No. You need to have that medical need noted on your reservation and WDW will accommodate you with that. Maybe not a specific building, but they will make sure you have what you need.

I've used online check in a few times. For our trip last Dec (stayed at POP) I was able to request ground floor and close to bus transportation. For our Feb trip (stayed at POP again) we didn't have any options because we had previously requested a king size bed.

If it's a medical condition where you need a room blocked, then you should go through special guest services. Otherwise, room assignors can only note your request and hopefully CM's won't make changes at check-in.
