We are on the DDP and I can't eat sugar, so desserts are out. I'm wondering if they do any substitution for dessert at TS restaurants... like an appetizer or something? Thanks!

No, there is no substituting appetizers, but the chefs will work with you on finding something you can eat. There are sugar-free dessert options available. Just note on your ADRs that you have an issue with sugar and the chef will come out and speak with you.

I positive that every menu has at least one sugar free desert option on it.

My dad is diabetic and he always just asked for a sugar free option. There was one available everywhere he asked.
