Well, I cheated, and Googled "Goofy Golf Course", and came up with this:

"Goofy Golf in Fort Walton Beach, FL. has been operating since 1958."

    and:

    "GOOFY GOLF
    3020 Milan Road
    Sandusky, OH 44870"

BUT - as it relates to Disney , it's on the Disney Dream and Fantasy cruise ships
