So all out F+ reservations are kept on our bands? What do they do, scan our bracelets when we get into the FP lines?

I think it is actually your FP+ info is kept on Disney's servers... and you band is simply an RFID identification tag that says to a computer who you are. Then it matches up your personal ID with the info sorted on the main servers. if the FP+ is logged for that time for your name... Mickey says "go".

John is correct.

All information is actually stored on Disney's servers. This includes resort reservation info, room assignment, credit card info, tickets, ADRs and FP+. Nothing is actually "ON" the bands. The band just has an RFID chip in it that contains a serial number. That number is associated with your account and is used to access the data.

When you first enter the FP+ line, you touch your band to the Mickey head. If you have a valid FP+, Mickey turns green and you are allowed into the queue. Prior to actually getting on the ride, your band will be scanned once more. Again, green means go.

What's the purpose of the second scanning?

You scan a second time at the end of the line to ensure that people aren't "jumping" into the queue without a fastpass between the entrance to the ride and the end of the line.
