Link to picture of fastpass+ cards that they give at each park

Can anyone share a picture or possibly a link to see the fastpass+ "choice cards" that you can get at each park when you are making your fastpass+ choices at the kiosks?

This isn't exactly what you're looking for, and I couldn't find a picture of the cards, but I did find a super list of the FP+'s available at each park.
