How does the whole baby swap thing work?

My family and I are planning on making a trip to WDW next summer (2013) when our youngest is almost two years old. I've heard about the "baby swap" option on many of the rides but i'm curious how does it work exactly? Also, is it available on all rides that a two year wouldn't be able to go on?

If your child falls below the required heighth requirement, one of you goes into the regular queue line, while the other gets a "child swap pass" once the first person rides the ride, they return and the other person uses the child swap to go into the fast pass line (you can bring 2 other people)
