Never been there but can tell you it is located on the 2nd floor above Chefs De France, I think the entrance is behind Chefs.
The entrance is behind Chef's, 1st door on the left as you turn onto the street that the pastry place is on. I'm not sure, but I think the dress code is the same as any other theme park restaurant, however, when we ate there everyone I saw had at least nice shorts and collared shirt/blouse or dress or better.
It was great. everything about it. The best restaurant I've ever eaten in, but I don't get out much so who knows. ;)
It is also (I belive) the second most expensive restaurant at Disney (although Shula's was pretty expensive.)
Also, don't have anything with a set time planned afterward. They really take their time (in a good way) and don't rush you. You will be there for awhile. Enjoy.

We ate there in February after a gap of many years. the food, service and presentation could not be faulted. The wine was not as outrageously overpriced as it had been. We got tempted to 3 courses and I think it was about $200 for the two of us. Not a price we would ordinarily pay buy great for a special occasion - I have booked to go again on my 'official' 50th birthday later this year.
We ate there about 2 years ago and it was amazing! Definitely one to try, but be prepared to spend over $50 per person.
I'd recommend trying to snag a table by a window during Illuminations

oh yes, on ours that price did include a kir each to start with, a bottle of wine and a digestif!

This place is completely romantic, the food is delicious and if our waiter is representative of the others, has a great and very helpful staff.
I agree about getting a window by illuminations, Plex and I had a great time watching the fireworks!

Bistro de Paris does not participate in the dining plan, but it is absolutely a signature restaurant considering the price, atmosphere and menu. Bistro is considered one of the elite restaurants on property.
I admit I have never eaten there, so I cannot speak from personal experience, but I know Bistro enforces a dress code and does not welcome children. For people interested in a leisurely, gourmet dining experience, I have rarely read a negative review.

Bistro De Paris participates in the Tables in Wonderland program, but not the Disney Dining Plan.
I have eaten there and enjoyed my meal. To be honest, I did not think that the food, though different choices, tasted any better there than when I have eaten at Chefs De France (I enjoy both restaurants). This makes absolute sense, though, as it is the same kitchen preparing the food for both restaurants.
What make Bistro a great experience is the personal service you receive there due to the small staff to guest ratio. This is why you pay more for your meal. It allows for you to have individualized attention and enjoy a more leisurely meal. There isn't a kid's menu and children are not encouraged here.

They do allow children, but there is no children's menu and it's not child friendly. The only restaurant that limits children is Victoria and Albert's (which is the most expensive on property, well over Bistro and Shula's). V & A will not allow children under 11. Bistro just politely discourages them, but does allow them in. Knowing that, it's not a place I would bring my children (Chefs de France is great and family-friendly as an alternative), but to each her own. Bistro, like V & A, is Disney Signature Dining that is not on the dining plan.
