When ordering CS at CSR are there to go type boxes and do you have to pay gratuity for to go?

No, you don't have to pay for grat on any CS, regardless of if it's to go or not.

They will have lids for you to use if you wanted to take it back.

I think what the OP is asking about is the extra fee that CS adds to it's food court dining. To my knowledge (and we have stayed there on our last two trips) the extra fee is waived for to go food.
